Five photos and one yes, while you’re there
Print this or keep it on your phone. It takes one afternoon, and you don’t need an account to use it.
The five photos
- Every pill bottle, label facing you, including the ones in the bathroom and the bedside drawer
- The front of each insurance card, including the Medicare card
- The back of each insurance card
- Any discharge papers from a recent hospital or rehab stay
- The list of doctors, wherever it lives: the fridge, an address book, a note by the phone
Questions to ask Mom or Dad
- Who are your doctors, and which one do you see most?
- Which pharmacy do you use?
- Who holds your health care proxy?
- Where is your advance directive kept?
The one yes
- Ask them to say yes to connecting their records.
It’s their choice. They can say no, and they can change their mind later.
