Getting records together

Several doctors. Several portals. One place to make sense of the papers.

You finally get into the portal, find the visit and download a file. The next specialist uses another system. When someone asks what the earlier doctor said, the answer is somewhere among those downloads.

For a parent, spouse or sibling, or your own care. Inviting someone to the Care Circle is free.

You should not have to remember which portal held each piece just to prepare for a conversation. It helps to know what you have, where it came from and what is still missing.

Where Dekona can help

Turn the pile into information you can return to.

01

Start with one document

Use an authorized photo or upload before deciding whether to connect records.

02

Know the source

Keep explanations tied to the underlying information rather than relying on an unsupported answer.

03

Connect where supported

Available connections require authorization. Not every provider, record type or proxy situation is supported.

Illustrative handoff · Invented people and excerpts

Jordan is helping their spouse Taylor prepare for a visit.

Uploaded visit note · Sample excerpt

Follow-up with the specialist was discussed.

Shared note

Taylor wants to ask whether another appointment is needed.

  • The uploaded visit note and its original wording
  • The question to bring to the specialist
  • A visible gap where a record or confirmation is missing

What becomes clearer

The note records a discussion. It does not establish that a referral was placed or a visit was booked.

Jordan and Taylor can return to the source note before calling the specialist. They still need the office to confirm whether a referral or appointment actually exists.

An explanatory example, not a generated record or treatment instruction.

Before you begin

A few good questions.

Can I get records just because I am a relative?

Being a relative or friend does not itself grant record access. Use information you are authorized to share; connection options depend on your authority and the supported source.

Will every record appear automatically?

No. Coverage depends on supported sources, available information and authorization. Uploads can provide another starting point; review important gaps.

Who can see care information?

A referral or family relationship does not grant record access. Record connections and invitations require appropriate authority and scoped permission. Dekona does not contact doctors or pharmacies for you and does not replace your clinicians.
